Prenatal 0-5 Home Visits
The Home Visitation Program is committed to helping new parents face the uncertainties that come from raising a child. Registered nurses from the Health Department visit parents in their homes to give support, teaching and parenting information for infants who have risk factors which may include: prematurity, low apgar scores, birth trauma, substance abuse, and need for parenting skills. 

Medicaid/Targeted Case Management
This program is a Medicaid benefit for infants and families. Registered nurses make home visits to assess the needs of families and give referrals for community resources.

Someone from your local health department will call you to schedule a time for the nurse to visit with you in your home.
